The form is in Microsoft Word format to allow you to fill it in on your computer and email back to us as an attachment to enquiries@mnwb.org.uk. Please telephone the office to advise us that you have sent the application.
The Board will acknowledge receipt of all applications by reply email.
How to use the form
- Save a copy of the form to your computer and then fill it in for each application
- For each new application save the form with a different name so you don't overwrite previous applications
- Filling in the form is easy
- The titles and areas you can't change are locked to prevent errors
- The grey shaded areas indicate where a response is required, they will grow according to how much you have to say
- To tick a box simply click on it and a cross will appear
- Email finished forms to us at enquiries@mnwb.org.uk
